Documents Required For Birth Correction
Attach the following necessary documents for birth certificate correction in the application form:
- Aadhaa Card.
- Valid birth certificate.
- Education Certificates.
- Name of the Parents.
- Date of birth of the child.
- Location of residence / residence certificate.
- Local notary’s Affidavit.
- In case of name change, an Affidavit on a non-judicial stamp paper mentioning about the reason of name change.
- Aadhaar Card as well address proof such as Voter card, DL etc. of parents (if required).
Birth Certificate Correction Form
Before making the correction, you will have to show that the information originally provided at the time of the birth registration was wrong. Also, you will have to provide a copy of the valid birth certificate and supporting documents showing the correct information. Documents provided for correction should be valid or dated nearby the date of birth. In case you are unable to provide any evidence verifying the error having occurred in the certificate, then correction might not be possible.
Note: Self attested copies along with original documents should be presented with the application form as advised by concerned authority. Also you might have to provide any other documents / information, if required by the authorities for the processing.
The original information will always be shown in the certificate as it was the first information given at the time of the registration, but a note will be written in the margin of the register mentioning what the correct information should be and the date on which the correction was done.
