రేషన్ కార్డ్ దరఖాస్తు ఫారమ్ పిడిఎఫ్లో సభ్యుని తొలగించడం Ration card is a govt. issued document used by citizen of India to get the ration at subsidized rates. It can also be used as identity and address proof to prove that you are resident in particular state. It is very important document of person to own. You can easily add or delete member is your ration card as per the situation. Deletion of member from ration card is required in case of death of person and for other reasons.
Deletion of member from ration card can be done both by online or offline method as the applicant desires. To apply for deletion of member thorough offline process in Andhra Pradesh, applicant have to submit AP Ration Card Member Deletion Form (రేషన్ కార్డ్ దరఖాస్తు ఫారమ్ పిడిఎఫ్లో సభ్యుని తొలగించడం) to the concerned dept. along with required documents for the same. After verification by the dept., your application for deletion of name will be completed and your ration card will be updated.
(Note:- Beneficiary can delete name from existing Ration card. And can download online form and submit it with necessary documents to near Zonal / Mamlatdar office / Civil Supplies Dept in the state. But name of Head of family can not be deleted through this process).

Documents Required
- Completely filled & signed Application Form.
- Recent passport size photo
- Document proof for Member Deletion
- Mention the reason for deletion of member name.
- Affidavit from gazetted officer-stating reason for deletion of member (Not mandatory).
Offline Application Process
To apply for deletion of member from ration card, you have submit the application form for it to concerned dept. You can get the form from any nearby ration card shop dealer or form concerned dept. You can also download it by using the link provided here.

- Once you have downloaded the form, read it carefully and fill the required information correctly.
- Fist enter your ration card no., and then enter member details for deletion i.e. Name of member, DOB, your Relation with member and Action for deletion.
- Then enter informant details such as Informant Name, Reason for deletion for member name, Relation, and Mobile No, and mention Delivery Type ( Manual or Speed Post).
- Once the form is completely filled, attached the required documents with the form, sign and submit the same to your nearby ration card food supplier shop or to concerned dept.
- For deleting name of member in case of death, you will have to submit death certificate along with application form. If the person has recently been married and has decided to remove the name from the family’s ration card, they will have to produce a marriage certificate.
- Once you have submitted the form, you will be given acknowledgement receipt. You can track your application status online.
